Bleeding after sexual intercourse

" Bleeding during or right after having sexual intercourse is known as “postcoital bleeding” PCB in medical terms. This medical condition is not the same as the menstrual bleeding and must not be ignored."

Causes

There are numerous reasons that lead to vaginal bleeding, let us discuss a few of them below:

  • Cervical ectropion refers to the medical condition where cervical cells that otherwise form the inner lining of the cervix appear on its outer lining. This is either due to hormonal changes (Oral contraceptive pills or hormone replacement therapy HRT) or infection. Sometimes, there is no reason identified.
  • Abnormal endometrial tissue growth called endometrial polyp. This usually is benign and not cancerous.
  • Uterine fibroids that are benign (non-cancerous) tumours of the uterus. Sometimes when they are present within the cavity of the uterus or just outside, can cause PCB.
  • Certain sexually transmitted diseases such as herpes, gonorrhoea, and chlamydia lead to bleeding after sex.
  • Vaginal dryness (known as Atrophic Vaginitis) that occurs due to reduced vaginal secretions and lack of hormonal support after menopause
  • Inadequate lubrication due to lack of foreplay leads to friction and bleeding
  • Sexual intercourse just before or after your periods leads to normal uterine bleeding during or after intercourse
  • Cervical or uterine cancer may also cause vaginal bleeding after sexual activity.

Diagnosis

The doctor first enquires about the symptoms of vaginal bleeding in detail and takes full medical history. Diagnosis usually begins with a physical examination of the pelvic region which include abdominal palpation, speculum examination and transvaginal ultrasound scan.

Vaginal swabs from the vagina and cervix will be taken for further examination when infection is suspected.

The ultrasound scan is done to check for uterine fibroids, endometrial polyps or sometimes a cyst in the ovary.

Treatment

Treatment entirely depends on the cause and varies from patient to patient based on the precise diagnosis. For instance, if vaginal dryness is the cause of bleeding during or after sex, patient may just only be prescribed to use lubricating gels to overcome the issue.

If the patient has vaginal infection, the doctor may prescribe antibiotics for a short duration. On the other hand, uterine fibroids and polyps need surgical procedure or minimally invasive removal as an outpatient procedure. Further evaluation by CT scan and MRI and referral to cancer specialist is done when cancer is suspected.

All women between 25 to 64 years of age must get cervical screening tests (pap smear) done on a regular basis (every 3 years) to rule out the possibility of cervical cancer.
